    • To verify that you qualify for the scheme, please upload one of the following documents to confirm the child's date of birth:

      • Birth certificate; or
      • MATB1 certificate; or
      • Child's NHS Personal Child Health Record ("Red Book") showing their name and date of birth; or
      • Letter from a health visitor, GP or midwife confirming the child's date of birth; or
      • For foster parents or other carers, a letter or placement confirmation from the fostering service or local authority confirming that the child is in your care and their date of birth.

      Please also upload one recent document showing your address in Wychavon, such as:

      • Council Tax bill;
      • Utility bill;
      • Bank or building society statement;
      • Tenancy agreement; or
      • Other official correspondence showing your name and address.

      These documents are required to verify your eligibility. Unfortunately, we cannot process your application without them.

      • Cashbacks must be claimed by the end of the financial year in which you apply (i.e. 31st March)
      • Only open to children up to two years old
      • Applications must have the MAT-B1/birth certificate attached – applications without these documents will be delayed/not processed
      • Cashback can only be redeemed on the Nappy Lady website towards the purchase of reusable nappy products and cannot be used to purchase swim nappies, wipes, buckets/storage, detergent.

      Your understand that your information will not be shared with any other third parties, other council departments, or used for marketing purposes, or any other purpose, other than that connected with processing your application, unless you give your express consent.

      Any proof of identity, proof of address, or other supporting documents provided to verify your eligibility for the scheme will be used only to confirm that you are a resident of Wychavon and your child is under the age of two and therefore qualifies  for the scheme. Once these checks have been completed, copies of your supporting documents will be securely destroyed in accordance with the council's data handling procedures.
